As an emerging adult, you may wondered if you will always be cared for by pediatric clinicians…

As a patient or parent cared for by the Yale Pediatric Type 1 Diabetes Clinic Team, you will always be part of our family but we realize that as an adult you will have medical care needs that are better addressed by an adult diabetes care specialist. We created The Yale Bridge Clinic with the core goal of helping you smoothly transition from the care of pediatric clinicians to the care of adult clinicians.

At the Yale Bridge Clinic, you will see a physician who is trained in both pediatric and adult care. Dr. Ania Jastreboff is a board certified pediatrician and internist, with subspecialty training in both Pediatric Endocrinology and Adult Endocrinology. She will see you 2-3 times at the Yale Pediatric Type 1 Diabetes Clinic to begin the transition process and help guide you to an adult care provider. If you chose to continue to receive your care through Yale University, Dr. Jastreboff will then see you in the Yale Diabetes Center. At the Yale Diabetes Center, you will be acclimated to patient approaches in the adult-care setting, and introduced to the adult-care nurse practitioners and diabetologists in the clinic. 

Through the Yale Bridge Clinic, we will provide you with the support, guidance, and tools to inspire you to approach this part of your lives with excitement, and encourage you to independently care for your diabetes. We feel sure that you will emerge as an empowered adult!
